Go to the MAIDAM Zakat Application System:

Step 2:
Select "Permohonan Bantuan" (Aid Application)
Step 3:
Prepare and upload the required documents:
- Copy of applicant’s MyKad Copy of spouse’s MyKad
- Copy of father’s/guardian’s MyKad
- Copy of mother’s/guardian’s (or spouse's) MyKad
- MyKad of all household members
- Applicant’s income statement
- Spouse’s income statement
- Father’s/guardian’s income statement (for child applicant)
- Mother’s/guardian’s spouse’s income statement (for child applicant)
- Medical confirmation letter from a government medical officer (for parents/guardians)
- Conversion to Islam card (for converts)
- Quotation from vendor (for medical equipment or medication)
