Application can be made via portal https://stdi.terengganu.gov.my/mohon.php or through Sekretariat Tabung Darul Iman office
Applicant need to provide the following documents:
1. Copy of Birth Certificate / IC of applicant, parents or guardian.
2. Proof of Terengganu citizenship or residency (if applicable).
3. Proof of voter registration in Terengganu.
3. Verification letter:
From government doctor/specialist (for medical aid).
4. Copy of patient’s bank account (and nominee, if any).
