1. Application can be made at Bahagian Zakat dan Fitrah MUIS office at Tingkat 1, Blok A & B, Wisma MUIS
2. Applicant need to bring the following certified documents :
- Copy of Applicant’s Identity Card
- Copy of Salary Slip / Income Verification / Statutory Declaration of Income (Applicant/Spouse)
- Copy of Applicant’s Bank Statement
- Copy of Conversion to Islam Certificate / Muallaf Card (if applicable)
- Copy of Marriage / Divorce / Death Certificate (if applicable)
- Copy of Hospital Verification Letter, Hospital Appointment Card and Medical Report
2. Applicant need to bring the following certified documents :
- Copy of Applicant’s Identity Card
