Where to Apply

Step 2: Obtain Doctor’s Support Letter
Get a *validation and referral letter* from the *treating doctor*.
Step 3: Attach a recent medical report* from the same treating doctor.
Step 4: Send the *completed form and documents* to the *Jabatan Kerja Sosial Perubatan* at the treating hospital.
Step 5: Hospital Submit Application
The application will be officially submitted to MAIPs by the hospital’s Social Work Department.
Step 6: Interview by MAIPs
Patient or caregiver will be contacted for an *interview session* by the MAIPs team.
Step 7: Notification of Result
If approved, the *Jabatan Kerja Sosial Perubatan* will notify the applicant via phone call.
Step 8: Aid Disbursement
The aid will be channelled via cash or cheque to the patient or caregiver.
