Step 1: Download the Application Form

*Step 2: Get the Form Verified
Bring the form to your *treating doctor* for verification. Also get it endorsed by respective community leader* (e.g., Ketua Kampung or ADUN office).
Step 3: Submit the Completed Form. Send the form along with the required documents to the Jabatan Zakat, Majlis Agama Islam Negeri Sembilan (MAINS).
Step 4: Attach These Documents.
Make sure to include the following:
1. Copy of *applicant’s IC*
2. Copy of *spouse’s or guardian’s IC*
3. Copy of *children's IC or birth certificates*
4. Copy of *marriage/divorce/death certificate* (if relevant)
5. Copy of *latest water or electricity bill*
6. Copy of *pay slips or income verification* (applicant & spouse)
7. Copy of *bank book or bank statement*
8. Copy of *EPF (KWSP) statement* (if any)
9. *Quotation* for medical treatment (if applicable)
