1. Obtain the Application Form at SKE office
2. Complete the Application Form
3. Fill in all required details.
4. Have the form endorsed by the patient’s doctor.
5. Attach Supporting Documents that Include a certified copy of the latest medical report from the hospital or medical centre.
6. Submit and return the completed form along with all supporting documents to the office.
7. Successful applicant will be called for an interview by SKE fund committee team at patient home and treating hospital
