3. The household income does not exceed the poverty line set by the state of Melaka [Not more than RM840.00 per month for a family of 4: per capita RM210.00 (as of 2013)].

1. Obtain Application form at MAIM Office, Bahagian Agihan Baitulmal
2. Fill In and attach medical report and document required: 2 passport-sized photos of the applicant, a copy of their IC, divorce certificate (if divorced), death certificate (if widowed), applicant’s birth certificate (if available), and birth certificates or ICs of each dependent child.
3. Get endorsement by community leader, or local religious leader in applicant resident
4. Submit the complete document to Nearest Pusat Agihan Baitulmal under MAIM or Bahagian Agihan Bailtulmal in MAIM
