Where to Apply
Step 1: Patient can inform their doctor about the need to apply for the aid.

Step 2: Doctor will help to refer patient to Medical Social Work Department (JKSP).

Step 3: Patient will attend appointment at JKSP with the documents (Medical Report Treatment Form, Dietetic Form - filled by Dietetic Officer, IC and PET / CT Scan Request Form).

Step 4: Interview will be conducted with patient.

Step 5: Application form of patient will be sent to TLJCF for processing.
